"MS is always in the back of your mind. If there is something you want to do, you always wonder if the MS will allow you do to it." —Darlene, living with MS for 22 years Living with multiple sclerosis (MS) is challenging and multidimensional. MS pervades all aspects of life: one’s body becomes unpredictable and unreliable, one’s identity and sense of self are tested, and relationships with others often change. MS symptoms emerge and remit; limitations evolve and progress. MS rehabilitation is an active, person-centered, and goal-oriented process embedded within a respectful and collaborative partnership between the person with MS and the members of his or her rehabilitation treatment team. Using the International Classification of Functioning, Disability and Health (ICF) as a guiding framework, Multiple Sclerosis Rehabilitation: From Impairment to Participation provides a comprehensive and evidence-based resource to inform and guide clinical reasoning and decision making during each phase of the MS rehabilitation process, from initial referral to post-discharge follow-up. With an emphasis on the application of evidence throughout the entire MS rehabilitation process, the specific objectives of the book are to increase the understanding of: The nature and impact of specific impairments, activity limitations, and participation restrictions experienced by people with MS How to select and use valid, reliable, and relevant assessment tools to inform the development of rehabilitation goals and intervention plans, and to evaluate outcomes This book provides information about the nature and impact of MS on the daily lives of people living with the disease, describes evidence-based assessment processes and instruments, and summarizes current knowledge that can inform goal setting and intervention planning. Thoughtful application of the knowledge contained in this book will inform and guide rehabilitation providers to work collaboratively with people with MS and enable them to achieve their goals for participation in everyday life.

Recent years have witnessed dramatic advances in the development and use of magnetic resonance imaging (MRI) techniques that can provide quantitative measures with some degree of pathological specificity for the heterogeneous substrates of multiple sclerosis (MS). Magnetic resonance spectroscopy (MRS) is one of the most promising of these techniques. Thanks to MRS, axonal damage is no longer considered an end-stage phenomenon typical of only the most destructive lesions and the most unfortunate cases, but rather as a major component of the MS pathology of lesions and normal-appearing white matter at all the phases of the disease. This new concept is rapidly changing our understanding of MS pathophysiology and, as a consequence, the therapeutic strategies to modify the disease course favorably. Many of the authors have pionereed the use of MRS in MS, thus contributing to the foundation of the "axonal hypothesis".

A synthesis of current concepts about the evaluation, treatment, and future directions in MS. On the evaluation side, the authors review the use of MRI, magnetic resonance spectroscopy, functional MRI, and three-dimensional MRI, and consider the rapidly developing body of pathologic information they have yielded. On the treatment side, the focus is on recently approved medications (Novantrone), new indications for medications (CHAMPS Trial), medications in development (Oral Interferon Tau, Oral Copaxone, and Oral Cellcept), immunosuppressive therapy for both progressive disease and symptomatic therapy; the current medications for treating relapsing-remitting MS (Avonex, Betaseron, and Copaxone) are also discussed. For future directions, the authors present the current best thinking, as well as the latest discoveries in immunology relating to MS, including groundbreaking B-cell research and its applications to specific immunotherapies, and the use of immune markers for tracking the disease.

Jen Powley was diagnosed with multiple sclerosis at fifteen. By thirty-five, she had lost the use of her arms and legs. Just Jen is a powerful memoir that tells the story of Powley’s life at the time of her diagnosis, and the infinite, irrevocable ways it has changed since. Powley’s writing pulls no punches. She is lively, bold and unapologetic, answering questions people are often afraid to ask about living with a progressive disease. And yet, these snapshots from Powley’s life are not tinged with anger or despair. Just Jen is a powerful, uplifting and unforgettable work by an author who has laid her life — and her body — bare in order to survive. Overcoming Multiple Sclerosis is an established and successful program of treatment. Once a diagnosis of MS meant inevitable decline and disability. Now thousands of people around the world are living healthy, active lives on the Overcoming Multiple Sclerosis recovery program. Overcoming Multiple Sclerosis explains the nature of MS and outlines an evidence-based 7 step program for recovery. Professor George Jelinek devised the program from an exhaustive analysis of medical research when he was first diagnosed with MS in 1999. It has been refined through major ongoing international clinical studies under Professor Jelinek's leadership, examining the lifestyles of several thousand people with MS world-wide and their health outcomes. Overcoming Multiple Sclerosis is invaluable for anyone recently diagnosed with MS, living with MS for years, or with a family member with MS. It makes an ideal resource for doctors treating people with MS. 'I would have no hesitation in recommending Overcoming Multiple Sclerosis to my patients, but also to my friends and colleagues.' Professor Gavin Giovannoni, MBBCh, PhD, FCP (S.A., Neurol.), FRCP, FRCPath, Chair of Neurology, Blizard Institute, Barts and The London School of Medicine and Dentistry 'Overcoming Multiple Sclerosis combines hard scientific evidence with practical advice and compassion. It will be of benefit to nearly everybody affected by MS and I heartily recommend it.' On this occasion we look back on 25 years of the Deutsche Gesellschaft fur Neuro chirurgie. They hold a great deal: founding and development of the society, comple tion and extension, communication between the individual members and contacts to other societies beyond our borders. They also stand for close co-operation with those who transfer their patients to u- the neurologists and specialists in internal medicine, the ophthalmologists and ear nose-and throat specialists as well as the general surgeons. This 25th annual meeting will deal with two examples of diseases that present common problems to the neurologist and to the neurosurgeon, namely meningiomas and multiple sclerosis. In view of their long histories often going back over many years, both of these lesions lead to diagnostic errors and indequate treatment. And yet it should be possible to recognize meningiomas at an early date and to initiate the only possible treatment, the operation,if all diagnostic measures are repeatedly carried out. The diagnosis MS, on the other hand, with the multiplicity of symptoms which are peculiar to this disease, should continue to be re-examined until every other lesion has been excluded with certainty. The increasing number of legal proceedings because of diagnostic and therapeutic measures as well as the doctor-patient talk preceding the written consent for these measures are further problems in need of discussion. For this reason, the topic "medical liability in special reference to the neurosurgeon" was chosen for this meeting. Many questions necessitate many answers.

P.A. MURARO, A. LUGARESI, D. GAMBI Many of the pathological aspects of multiple sclerosis (MS) lesions have been known for over a century. It is only recently, however, that different patterns of demyelination have been linked to distinct pathways of immune-mediated tissue destruction. In particular, the inter-individual heterogeneity of MS lesions has suggested that different mechanisms may act in different patients, accounting for the variability observed in clinical course, immunological findings in peripheral blood and cere brospinal fluid (CSF), and response to immunomodulatory treatments. To provide an overview of the basic mechanisms possibly involved in MS lesion initiation and development, an international meeting was organized in the context of the annual Congress of the Italian Neuroimmunology Association (AINI), held at the University of Chieti, in Chieti Italy on 29 October 1998. The high standard of presentations prompted us to report them in extended form, to highlight recent pro gress in the understanding of basic mechanisms sustaining MS immuno pathogenesis. A central role in the possible mechanisms leading to myelin destruc tion has been attributed to T lymphocytes reactive to myelin antigens. Studies on the myelin antigen-specific T cell repertoire have contributed significant advances to our knowledge of autoimmunity (Chapters 1,2).

In a series of dramatic essays and photographs by the renowned San Francisco-based photographer Amelia Davis, My Story is an evocative description of what it is like to live with multiple sclerosis (MS), a disease that affects about 350,000 Americans and whose cause is still not entirely known. The essays and accompanying photographs in this highly engaging, beautifully illustrated book poignantly portray the lives of thirty-two men and women from the ages of seventeen to seventy and of various ethnicities, and socio-economic backgrounds who share the challenge of living with MS. Some, like Amelia, use no mobility aids, while others use canes, wheelchairs, or electric scooters. All have had to face the challenges and limitations that MS has imposed upon their lives, and each has devised unique and often creative coping strategies.Accompanying each essay are commentaries by family members and friends that express their own personal feelings and experiences of living with this disease. Here are the inspirational stories of women, men, and children who live with this disease. Many have children; one or two are currently expecting them. Treatments and therapies for slowing the progression of the disease are revealed and shared, from the latest advances in prescription medication to alternative methods of coping, including yoga, exercise and competitive sports, creative activities such as writing and art, and even community activism. A wide range of accompanying stories by spouses, children, and other loved ones depict the ups and downs of living and caring for someone who has MS, from the moment of first diagnosis to dealing with its ongoing challenges. All are strong reminders of the selflessness of the human spirit, and its ability to nurture and remain strong under even adverse circumstances. Highly motivating and deeply inspirational, My Story will be welcomed by anyone who lives with or shares the life of someone who has MS.
