Fungal diseases have been with us from antiquity; interest in the chemo therapy of fungal disease has exploded in the past decade. To plan and pro duce a book on the topic of antifungal chemotherapy has come as a personal challenge - and something of an eye-opener - towards the end of my re search career. A landmark publication which still merits reading is Antifungal Chemotherapy (John Wiley & Sons, Chichester, UK), edited by David Speller, which appeared in 1980. However, the fact that ketoconazole, the first of the modern, orally active, wide-spectrum antifungals, attracted no more than two sentences in it indicates just how far we have come in the 1980s. A steady stream of original papers and a number of conference proceedings have chronicled this progress in drug research; outstanding among the latter are the proceedings of an international telesymposium, entitled Recent Trends in the Discovery, Development and Evaluation of Antifungal Agents, edited by Robert Fromtling (J.R. Prous, Barcelona, 1987) and volume 544 of the Annals of the New York Academy of Sciences, entitled Antifungal Drugs, edited by Vassil St. Georgiev, and containing papers and posters presented at a most enjoyable 3-day conference held at Garden City, New York, in the autumn of 1987.

Fungal infections such as candidoses can range from superficial mucous membrane infection to life-threatening systemic mycoses. Candida infections are a significant clinical problem globally due to rapid rise in compromised host populations including HIV/AIDS, organ transplant recipients and patients on chemotherapy. In addition, sharp increase in aging populations which are susceptible to fungal infections is expected in next few decades. Antifungal drugs are relatively difficult to develop compared to the antibacterial drugs owing to the eukaryotic nature of the cells. Therefore, only a handful of antifungal agents are currently available to treat the myriad of fungal infections. Moreover, rising antifungal resistance and host-related adverse reactions have limited the antifungal arsenal against fungal pathogens. In this research topic, we tried to update the theoretical aspects pertaining to the antifungal drug discovery i.e. proposed novel mechanisms, new drug targets and pathways. In addition, invited authors explored the new antifungal drugs derived from natural and synthetic sources which are currently under development. Ojectives: Fungal infections represent an increasing threat to an expanding population of at risk patients; but the impact of these diseases on human health is not widely appreciated. The mortality associated with some of these infections remain unacceptably high. Because fungi are eukaryotic, there is a paucity of selective targets that can be exploited for antifungal drug development. As a result, the current antifungal armamentarium is very limited. Furthermore, the antifungal drug pipeline is mostly dry. There is no doubt that novel antifungals and new approaches to develop them are urgently warranted. Unlike the costly and time consuming process of de novo drug discovery, drug repurposing is a novel method to reduce the time, cost and risk associated with drug innovation. Methods: The present studies build upon the recent discovery by our group of the potent antifungal activity of FDA-approved drug Auranofin, which we originally identified in high content screenings of the Prestwick library against Candida albicans biofilms. MICs were determined in accordance with the CLSI M27-A3 (for yeast) and M38-A2 (for filamentous fungi) reference standards for antifungal susceptibility testing. We also performed in-vivo evaluation of these drugs in the murine models of systemic cryptococcosis and hematogenously disseminated candidiasis Results: Screening the same library against other pathogenic fungi resulted in the identification of Ebselen as a leading compound displaying potent antifungal activity against different fungi. We observed increased survival of mice in groups treated with Auranofin and Ebselen compared to mock treated groups. Both drugs are organometallics and have many advantageous qualities including oral bioavailability, potent microbicidal activity in a clinical range and very low frequency of resistance Conclusion: In conclusion, repositioning old drugs can significantly decrease the time and effort that it takes to develop novel antifungal therapeutics, which represents a pressing and unmet clinical need due to the devastating nature of fungal infections. Thus, the present study seeks to provide further valuable evidence that both drugs have significant promise to be repositioned as novel antifungals for the treatment of fungal infections.
